• Connects uninsured patients with volunteer physicians

Patients must:

• Be uninsured
• Earn no more than 300% of the Federal Poverty Level
• Be referred by a medical home (primary care clinic or physician) that can provide ongoing care

• ✓  Emergency-only Medi-Cal qualifies
• ✗ Full-Scope Medi-Cal does not qualify
• ✗ Medicare does not qualify
• ✗ Private or other health insurance does not qualify 

Patients can be referred by a primary care home, or by a physician.
